Social Services Coordinator

  • Develop assessments and care plans to identify resident needs
  • Ensure care plan components are followed by reviewing MDS's and nurses' notes and attending meetings
  • Communicate patient information effectively with other departments and agencies
  • Participate in discharge planning and provide information to residents and families about available programs

Responsibilities:
  • Develop personalized assessments and care plans for residents to ensure their unique needs are being met.
  • Ensure all aspects of the care plan are being followed by reviewing nurses' notes, MDS's and attending meetings.
  • Collaborate with other departments and government agencies to communicate important information concerning residents' conditions.
  • Participate in discharge planning, providing information on programs and services available to residents and their families.
  • Keep up-to-date with the latest regulations and make recommendations on policy and procedure changes to ensure we provide the highest level of care.
Qualifications:
  • Bachelor's Degree in Social Services, Master's Degree preferred
  • 2-5 years' experience in social services in a Skilled Nursing or similar healthcare setting
  • Knowledgeable of regulations governing nursing care facilities
  • Understanding of Medicare, Medicaid, and private insurance
  • Excellent communication skills, both oral and written
  • Ability to maintain confidentiality of resident care information in accordance with HIPAA guidelines
